Manhattan Life offers Fixed Benefit health plans in 38 states. The availability of each plan(s) vary by state. Below you will find more information about these plans and the states where they are available.
As discussed on our NAHP Alternative Health Plan Solutions page, Fixed Benefit plans work differently than traditional major medical plans (like what are available under the ACA/Obamacare). With a Fixed Benefit plan, you receive a "fixed" payment from the insurance company for every covered health care service. The amount of each payment depends on the coverage level you choose and the type of health care service you receive.
Unlike major medical plans that have (often high) annual deductibles and coinsurance that you have to pay before the insurance begins paying, Manhattan Life offers Fixed Benefit plans that have NO deductible!

Manhattan Life Affordable Choice Plan Features and Benefits
The Affordable Choice plans provide the best overall coverage of any fixed benefit plan on the market for the MOST expensive type of care – Inpatient Hospitalization. The Elite Plus plan provides the highest benefit payout for inpatient hospital stays of any fixed benefit plan that I have found in the market.
Here are some of the features and benefits of the Affordable Choice plans:
- Male & Female rates are the same
- No increase in rates for tobacco usage
- Child only coverage starting after newborn’s 6-week exam
- Continuous coverage with stable rates from one year to the next (Unlike traditional STM that renews each year)
- No waiting periods for any coverage’s and coverage can begin next day (1st through 28th of the month)
- Pre-existing conditions covered after first year
- $5 million lifetime max per insured (Like the best major medical plans pre-ACA)
- No Deductibles, co-insurance or co-pays - First dollar coverage that can pay in excess of your actual medical bills to be used for other expenses
- Up to $8,000 first day/year hospital room & board coverage and $5,000 each additional day to $1 million max. per year (Elite Plus)
- Surgical Schedule pays up to 300% of Medicare schedule for surgeon’s surgery fee (Elite Plus)
- Up to $3,000 per day for surgery in an Ambulatory Surgery Center (in addition to surgeon benefit above)
- Up to $2,000 per day cancer treatment benefit (up to 40 days per year)
- Doc visits, Rx, preventive and many other healthcare services covered

Affordable Choice Enhanced NEW! - This is their most current and best overall coverage and value plan. It pays the same higher number of days for doctor office visits as the Affordable Choice 20 plan and significantly increases payment for most covered outpatient benefits. The Affordable Choice Enhanced plan offers 4 levels of coverage to choose from:Classic, Classic Plus, Elite and Elite Plus.

Preferred Provider (PPO) Network
The Manhattan Life plans allow you to go to any provider without restriction. However, the Fixed Benefit plans also include a nationwide PPO network. If you see in network providers, you are going to see significant discounts on the bills you receive.
NEW! Effective October 1, 2020, Manhattan Life will be transitioning their PPO network to the First Health nationwide PPO Network. All new policyholders whose coverage started near or after October 1st, will utilize the First Health network.
The First Health PPO Network. It is one of the most extensive PPO networks in the country. The First Health network provides access to:
- More than 5,900 hospitals
- Over 125,000 ancillary facilities;
- More than 810,000 physicians and healthcare professionals with over 1.5 million service locations across all 50 states.
• Network doctors that are carefully selected to promote quality outcomes.
• No paperwork because network doctors and hospitals file claims for you.
To look up providers go to the First Health PPO network link. It is available 24 hours a day, 7 days a week. It includes the most detailed provider information available. And, it’s kept up-to-date.
Start by clicking the dot next to "First Health Network." Then click the button that says, "Start Now." On the next page choose the type of provider and enter your search parameters. Then click the “Search Providers” button.

Included with the Manhattan Life plans is the Teladoc benefit at no additional premium or copay to use if you are sick. This telemedicine benefit will save you a ton of money and time from not having to go to the doctor’s office when you have a simple illness like cold or flu. You will receive information in your policy or you can call 1-800-TELADOC. Physicians are available by phone or Skype 24/7/365. You have unlimited use of this benefit at no cost.
Prescription Discount Program:

Manhattan Life Supplemental Benefits
Manhattan Life Enhanced Accident Expense Plan:
This coverage provides immediate cash of up to $4,000 to cover the costs of an accidental injury. You can also add disability benefits of up to $2,000/month for up to 24 months if you are disabled as a result of the accident. This can be written as a separate policy or added to one of the Fixed Benefit policies. (varies by state).
This plan also provides the following benefits for services related to an accident:
- Hospital inpatient benefit of $300/day
- $100,000 Accidental Death Benefit
- Up to $10,000 Ground or Air Ambulance Benefit
Enhanced Accident Plan Rates
Manhattan Life Out of Pocket Protection plan:
This is a VERY inexpensive supplemental plan designed to provide additional coverage for the first day hospital/ICU stay that you have in any year.
You choose a lump sum of $2,500, $5,000 or $6,350 for your first day in the hospital for each plan year. It also includes $100 or $200 per day to cover the cost of your doctor visiting you in the hospital. You can also add an optional (recommended) $250 Emergency Room Accident benefit rider and a rider that pays $1,000 to $3,000 for up to two Outpatient Surgeries per year.
Important Maternity Tip: Most women will deliver their baby in a hospital. That means they will ALWAYS have at least ONE overnight stay for the delivery. They will also have potentially LARGE out of pocket expenses for health insurance deductibles and coinsurance. As long as the mother-to-be is covered on this plan for longer than 1 year before delivery, the plan WILL PAY the selected benefit amount for the first overnight stay related to the delivery. Potentially saving you THOUSANDS out of your pocket.
ELIGIBILITY: The Out of Pocket Protection plans are available for people ages 18 through 69 (64 in California).

Manhattan Life Critical Protection and Recovery
Manhattan Life Critical Protection and Recovery plans include four different options that provide coverage for a variety of critical illnesses. Each plan option can be purchase with or without coverage for cancer.
These plans are different than other traditional critical illness plans that pay a one-time lump sum of $5,000 to $20,000 upon diagnosis. In addition to an initial lump sum payment upon diagnosis, these Manhattan Life plans pay an additional monthly benefit of 10% of the initial lump sum amount for 12 months after the initial payment.

Manhattan Life Cancer Care Plus Cancer and Dread Disease plan:
This is a different variation on most critical illness policies that pay a one-time, lump sum amount. It provides benefits for many cancer and dread disease related treatments. However, it also includes a unique MONTHLY Chemo, Radiation AND Immunotherapy benefit of $2,500 to $7,500. It also provides treatment benefits for many other dread diseases AND benefits for over 20 other related services.
ELIGIBILITY: Cancer Care Plus plans are available for people ages 18 through 69 (64 in California) and guaranteed renewable FOR LIFE!
Also, the monthly premium for your Cancer Care Plus plan is locked in based on your age when you sign up. The only way your premium will change is if the company raises the rate for ALL policyholders in your entire state.

Manhattan Life Dental, Vision and Hearing Insurance:
Manhattan Life Dental, Vision and Hearing (DVH) plan has three options with annual maximum benefits on the dental plan of $1,000, $1,500 and $3,000.
DVH plans are available for people ages 18 through 85
- Choose your dentist - In network or out of network (Plans use Careington Maximum Care PPO Dental Network)
- Family Rates (includes a maximum of 3 children)
- $1,000 - $3,000 policy year benefit option available
- Guaranteed Issue
- Guaranteed renewable for life (Subject to company's right to change premiums)
Available States (as of 10/19/2020): The $1,000 and $1,500 dental maximum benefit plan options are available in all states that the DVH plans are offered. The new $3,000 annual dental maximum benefit plan is available in these states:
