About LifeWise Health Plan of WA Health Insurance Plans
LifeWise Health Plan of Washington (aka LifeWise) is a member of the Premera Blue Cross family of companies headquartered in Washington state, with operations in Mountlake Terrace and Spokane, Washington; Portland and Bend, Oregon; Anchorage, Alaska; and Scottsdale, Arizona.
LifeWise) offers ACA/Obamacare individual/family plans ON the WA Health Plan Finder exchange. LifeWise plans are exclusive provider organization (EPO) plans, which cover the cost of care from in-network doctors, pharmacies, hospitals, and other care providers in Washington, Oregon, or Alaska. You can also get covered care from in-network providers without a referral. These plans cover care only from in-network providers, unless you have a medical emergency.
LifeWise offers their plans in 33 counties. In all counties they will offer their Essential plans (see below). Additionally, in Pierce, Snohomish and Spokane counties they will offer their Primary Gold and Silver plans OFF of the exchange.

LifeWise Provider Networks
The LifeWise Primary provider network offers access to major hospital systems, including Swedish Medical Center, Providence Health, and CHI Franciscan. You can also view their map of major providers statewide.

LifeWise Prescription Coverage and Formulary Information

All plans will use formularies M2 or M4. The Bronze and Catastrophic plans will use M2. All Silver and Gold plans will use M4.

NOTE: This dental plan is mostly for preventive and basic dental services. If you want more comprehensive dental coverage we offer plans from other companies. Just ask about them.
