Moda Health (aka Moda) offers ACA/Obamacare individual/family plans in Oregon and parts of Alaska. They also offer small group plans in Oregon and Alaska.

Moda State Availability
Moda offers their individual/family plans On and Off of the exchanges. On exchange plans are offered on the Federal Marketplace.

Individual Plans:
Benefit summaries for Individual and Family medical and dental plans offered in Oregon and Alaska are available on the Moda 2020 Plans page.
Small Group Plans:
On the Moda Employer Plans page you will find detailed information on all small group medical, dental and vision plans Moda offers in Oregon and Alaska. At the top of the page you can click the drop-down button to change the state.

Individual Plans:
On our Moda Individual Plans page,you will find information about Moda’s medical and dental plan provider networks.
To see if the providers you want are in their network for your plan and area, you can use the Provider Search Tool.
You can also get quotes and shop for plans. To begin, click the “Shop Plans” button anywhere on this page.You will first see a summary of the medical and dental plan networks. Next, enter your zip code and click the “Shop” button.Choose to shop for medical, dental or both types of plans and click “Continue.”Next enter the information about the person(s) who you want to cover and click “Continue.” On the next page you have the option to enter information on your annual income to see if you qualify for a tax credit subsidy. Next, click “See plans” to get your quotes.
Small Group Plans:
To find medical and dental provider network information go to Moda’s Group Plans Network page.
To see if the providers you want are in their network for your plan and area, you can use the Provider Search Tool.
Moda Prescription Coverage and Formulary Information
Individual Plans:
To see if the pharmacy providers you want are in their network for your plan and area, you can use the Provider Search Tool.
To search for specific medications, go to the Medication Search page. Existing members can login from this page to search the formulary for their plan. Non-members can choose the Individual/Family button and enter their zip code to be taken to the search page.
Small Group Plans:
On the Moda Pharmacy coverage page (included with each medical plan) you will find information on the pharmacy provider network and the prescription formulary of covered medications for all plans.
To search for specific medications, go to the Medication Search page. Existing members can login from this page to search the formulary for their plan. Non-members can choose the Small or Large Group button and enter their zip code to be taken to the search page.
